Abstract Chilaiditi syndrome is a rare condition defined by the presence of gastrointestinal symptoms associated to radiological finding segmental interposition large bowel between liver and diaphragm. Rarely identified as cause chronic abdominal pain, presents clinical significance it can lead number serious complications including intestinal obstruction, perforation, ischemia [1]. In this report we described case Syndrome due hepatophrenic ileal complicated an adhesive small obstruction (ASBO). Mininvasive surgery allowed diagnostic confirmation band adhesion entangling loop treatment obstructive without with good outcomes in terms reduced quick post-operative recovery short lenght hospital stay.
